Terry A. (Monica) Polniaszek, 45, of Brewerton, passed away peacefully on Wednesday, May 9th surrounded by her loved ones after a fierce fight with cancer.
Terry was a Registered Nurse dedicated to helping all of her patients. She was a devoted wife and mother.
Terry was predeceased by her father, Jack Monica.
She is survived by her loving husband of 12 years, Jeff; daughter, Pela; mother, Theresa Monica; three siblings, Anitra, Jackson (Heather) and Mike; niece, Ella; nephews, Addison, Ben and Trentan.
A memorial service will be held on Saturday, May 19th at 1PM at Northside Baptist Church, 7965 Oswego Road, Liverpool, NY 13090 with a reception to follow.
